Top Colors Fading

Looking to get some advise on how to combat this. Tried to search for an answer on the forum, but couldn't find exactly what I was looking for.

I almost always discharge my colors on dark garments, but I'm wanting to branch out to underbasing and printing Water Base or HSA on top. That way, when someone orders a blended garment, I don't have to try and sell them on the "vintage look".

 The photo attached shows one shirt that is right off the belt vs. one that has been washed. Here are the specs of the print:

-Gildan Ultra Cotton black tees
-Discharge White (CCI D-White) through 180 S Mesh. Low EOM. 3% Fixer and Softener. 5% activator. 70 Duro. 40 LPI
*FLASH* 4 Seconds
-Red Water Base (CCI W-Base with CCI pigments) through 158 mesh. Low EOM. 70 Duro.
-2:30 through Quartz dryer at 360 degrees.

I know that the obvious error was that I did not add fixer to the red, but even still, I feel like there should not be this much fading after just one wash test. During the wash test, I ran the print under water, lathered up some soap, and really went to town on scrubbing the shirt. Pretty rough, but not rough enough to justify this much color loss.

So my question is: Is this an adhesion issue, screen selection issue, ink issue, additive (or lack there of) issue, or something else? Any and all advice is welcome!

Or if someone can guide me to where this questions has asked before, that would be great too.

Don't use white as the ub. Too much pigment to hold anything over it.

Use the Premium Base straight or a mix of 80/20 base/white. CCI even makes
a product that is basically that mix.

Exactly!!  I like 85/15 base white. also you may want to hammer it in a little more doing a base and im not sure a 70d will do it with a 180

Makes sense! So are you saying to use a harder duro to drive the ink is further? What duro would you suggest? I forgot to mention, but I am printing this manually.
