Converting a CMYK color to a Pantone color site. (not physically in your program for you), but at least this tells you what PMS color a cmyk color might be close to. It's not exact but gets you in the window.

you can also do it in illustrator...

open the 'color guide' window/pallet...
select 'color books' from the bottom and choose Pantone Solid Coated (or whatever book you want)...

click on a color with the eyedropper tool

in the color guide window double click on the color that's closest to what you chose...

in the 'color' pallet window, you'll get the pantone name.

In photoshop or illustrator, I found it is really not accurate.

Back in the day, (at my old work) I had to match a lot of Madera embroidery threads to PMS colors and CMYK art to PMS colors.  I developed a nice long custom swatch list of all the matches. I kept that as I thought it would be useful to someone one day...but have no idea where it's at now.

I've done the technique in Illustrator but like Will35 said, it's not totally accurate. About as accurate as using photoshop to match a CMYK color and it gets to close but sometimes it's way way off.

Sometimes, it's not the program's fault. While we can get CMYK versions of Pantones, the reverse is sometimes not even possible to get close. They don't all have matches or even "first cousins".

I've always used the Photoshop method to at least get close, but I'm fairly surprised on occasion as to just how far away it is... sometimes the color the customer has in the file just doesn't have a Pantone match, or as Andy said, even a "first cousin".

Lets not forget the Pantone Bridge book is also available. . . It's not on the web, but I like using the book as you can look at may pages at the same time.
