When we first got into business I would try and print anything that would lay flat or I could fit on my press "that was my ad" and you know every hard job that all the smart printers didn't want I got.  I had only a manual press back then, and the one job that hit my door I wish never had, and guess what, 10 years later we are still doing that same job, on a manual, one pc at a time.  We have raise the price and increased the production time and they still use us, why? no body wants to F! with this job but my dumb azz.  Had one customer that came in the  shop and I guess she thought she own the place telling us how to move this customer and that so she could get what she wanted fast, so I did just what she wanted..gave her all her art on a cd and moved her on real fast.

Red flags include: 12 pieces and friend of friend. Those favor jobs always end up biting back.
We printed up a box of DC tees with our logo on them and often give them to customers unfamiliar with DC. They are unwashed and we suggest to them that they wash and wear it to see if DC is really an option for them. We can sell DC to them all we want but at the end of the day, that friend of a friend's wife's cousin's little sister says that she doesn't like them, you're going to hear about it. And hear about it. And hear about it. Add in all the headaches like you've mentioned and it can just become a real nightmare. Sometimes you just have to walk away!
