It sounds as though the Teflon is not adhered well to the screen. Am I correct?

A thin sheet of G-10 should work as well.

Well we tried it all, around the edges with extra strong tape or even with double sided tape covering the whole sheet.No matter what you run into trouble at one point due to it`s weight. Apart from that it`s a great product and we use it on almost every job.
This is meant to be constructive criticism and not slagging off a product so please don`t get me wrong here.

this is what I did with mine. Used the silver Duct tape meant for actual Ducts. Haven't had an issue yet in over a year. Same screen its been attached since the beginning. Like I said previously in the thread, I use just teflon sheet that I use for heat press, Actions was just too heavy for me to get it to stick on so I scraped it. I have a connection for Teflon so I cut a piece to fit my screen.
